2022 Application for Toys For Kids - deadline to apply is 11/25/2022 11:00 pm Central.

You must provide a copy of the following documents in order for your application to be considered submitted:

1.  Applicant's Driver's License

2.  Proof of Address
3.  Copy of EACH child's birth certificate listed on the application.

Attach the documents to your email or print out and bring with you when dropping off your application.
